    Food Allergies in School

    The following link provides information on the Food Allergy Guidelines as outlined in Public Act 96-0349 that was effective January 1, 2011
     

    If your child has a food allergy, please contact their physician to complete the following forms:

    Food Allergy Action Plan 

     
    Once these forms have been completed, you can contact the school nurse for your student’s building to discuss any accommodations or protocols that need to be implemented. 

    Allergies to Bee Stings:

    Asthma in School: 

    If your child has asthma, please contact their physician to complete the following forms:

     

    Once these forms have been completed, you can contact the school nurse for your student’s building to discuss any accommodations or protocols that need to be implemented.

    Seizures in School: 

    If your child has a history of seizures, please contact their physician to complete the following forms:

     
     
    Once these forms have been completed, you can contact the school nurse for your student’s building to discuss any accommodations or protocols that need to be implemented.

    Diabetes in School: 

    If your child has diabetes, please contact their physician to complete the following forms:

    If your child has an insulin pump or if you anticipate the use of one, all supplies must be brought to the Health Office prior to the start of the school year or before initiating pump therapy.

    With the proper authorization, we will administer Glucagon for severe hypoglycemia resulting in unconsciousness and/or seizures only.  The paramedics will be called if Glucagon is administered.

    Please contact your school nurse prior to the first day of school so that any accommodations and protocols can be discussed.
     

    Tube Feedings:

    If your child requires tube feedings while at school, please contact their physician to complete the following forms:
     
    Orders including the time of feedings, type of formula, route of administration and rate of administration.
     
     
    If your child requires tube feedings while at school, all supplies must be brought into the health office prior to the start of the school year.
