Please create an 8to18 account if you don't already have one for your student(s) and ensure your student(s) has a current IHSA Pre-Participation Examination Sports Physical ready when school begins. The Pre-Participation Examination Physical form can be found here. Doctor's offices also have them available during your appointment.

Please note, that an IHSA Pre-Participation Examination differs from the State of Illinois Certificate of Child Health Examination. Complete physical examinations and immunizations are required for entry into 6th grade. Health Requirements for all grade levels in SD308 can be found here.
