Authorization for Release or Exchange of Information
This is a PDF document that can be completed to allow CUSD 308 to release or share information regarding a student with outside agencies or organizations. Please complete this request and send the completed form to Aubrey Aguilera, Special Education Data & Student Records Specialist at the District Administrative Center.
