Overview

All Pre-K, Kindergarten, 6th grade, 9th grade students and students new to Illinois schools must provide a current physical exam and immunization record.  Students entering from another Illinois school must provide their most recent school physical and immunization record.  Exams must take place within one year prior to the first day of school.
 
All 6th grade students must show proof of having received all required vaccines including a Tdap vaccine, 2 doses of varicella and 3 doses of Hepatitis B vaccine at approved intervals.  One dose of meningococcal conjugate vaccine must be received on or after the 11th birthday.
 
All 12th grade students must provide proof of having received a dose of meningococcal conjugate vaccine on or after their 16th birthday.
 
Kindergarten, 2nd, 6th and 9th grade students are asked to provide proof of a dental exam dated within 18 months prior to May 15th of the current school year.
 
Kindergarten students and students new to Illinois schools need a vision exam.  Exams must be completed within one year prior to the first day of school and be submitted by October 15th.
 
It should be noted that a child's athletic physical assessment does not meet the sixth or ninth grade physical requirements. 
 
If you anticipate that your child may participate in school sporting events, have your physician complete a sports physical form while at your child's medical examination appointment.

Requirements by Grade

All students entering early childhood, kindergarten, 6th grade, and 9th grade are required to have a current physical examination completed on the approved Illinois Department of Public Health Form. A sports physical does not meet the requirement. A current physical is one that was completed after August 16, 2022. The parent portion on the upper backside of the form must be completed and signed.

Grade Requirements

Preschool
Complete Preschool Physical Examination
No additional immunization requirement if up-to-date with the following:
  • Diphtheria, Pertussis, Tetanus:  Three doses of DTP or DTaP by 1 year of age. One additional booster dose by 2nd birthday.  
  • Polio:  Three doses for any child 24 months of age or older appropriately spaced.  
  • Measles, Mumps, Rubella (MMR):  One dose on or after the 1st birthday.  
  • Haemophilus influenzae type b (Hib):  Proof of immunization that complies with the ACIP recommendation for Hib vaccination. Children 24-59 months of age without series shall show proof of one dose of Hib vaccine at 15 months or older.  
  • Invasive Pneumococcal Disease (PCV): Proof of immunization that complies with ACIP recommendations for PCV. Children 24 to 59 months of age without primary series of PCV, shall show proof of receiving one dose of PCV after 24 months of age.  
  • Hepatitis B:  Three doses appropriately spaced. (see doses under minimum interval). Third dose must have been administered on or after 6 months of age (168 days).  
  • Varicella:  One dose on or after 1st birthday.  
Kindergarten
Complete Kindergarten Physical Examination and all required immunizations including:
  • Two doses of MMR Vaccine
  • Four doses of Polio Vaccine
  • Four doses of DPT/DPTaP Vaccine 
  • Three doses of Hepatitis B
  • Two doses of Varicella Vaccine (chicken pox)
Complete Kindergarten Dental Examination - Must be turned in prior to May 15, 2024.
Complete Kindergarten Eye Examination - Must be turned in prior to October 15, 2023.

1st Grade
No additional physical/immunization requirements if up-to-date.

2nd Grade
No additional physical/immunization requirements if up-to-date.
Complete 2nd grade Dental Examination - Must be turned in prior to May 15, 2024.

3rd Grade
No additional physical/immunization requirements if up-to-date.
4th Grade
No additional physical/immunization requirements if up-to-date.
5th Grade
No additional physical/immunization requirements if up-to-date.
6th Grade
Complete 6th grade Physical Examination and all required immunizations including:
  • One dose of Tdap Vaccine
  • One dose of Meningococcal Vaccine (meningitis) after the 11th birthday
  • Two doses of Varicella Vaccine (chicken pox)
  • Three doses of Hepatitis B at the required intervals
Complete 6th grade Dental Examination - Must be turned in prior to May 15, 2024.

7th Grade
No additional physical/immunization requirements if up-to-date.

8th Grade
No additional physical/immunization requirements if up-to-date.

9th Grade
Complete 9th grade Physical Examination and all required immunizations.
Complete 9th grade Dental Examination- must be turned in prior to May 15, 2024.

10th Grade
No additional physical/immunization requirements if up-to-date.

11th Grade
No additional physical/immunization requirements if up-to-date.

12th Grade
Must show proof of receiving two doses of meningococcal vaccine (meningitis) with the second dose received on or after the 16th birthday. If the first dose was given after the age of 16, then only one dose is required.
